Travel
registrants
must:
1)
Submit a copy
of player's Birth Certificate (First time registrants ONLY).
2)
Provide a 1"
x 1" current photo of the player (Fall season only for returning
players for new soccer year player pass).
3)
Sign the
bottom of Registration form where indicated "Travel Player Only
Signature ".
